5-962    SUCCESSFUL APPLICANTS OF ORAL TESTS AND FIRST SEGMENTS OF TWO-SEGMENT FLIGHT TESTS. Applicants will use only one application for the practical test. Inspectors and examiners must complete the following actions when an applicant’s performance during an oral test or a flight test is satisfactory:

A.    Oral Test.

Indicates new/changed information.
1)     Mark, date, and sign the appropriate spaces of Federal Aviation Administration (FAA) Form 8710-1, Airman Certificate and/or Rating Application and FAA Form 8410-1, Airman Proficiency/Qualification Check or FAA Form 8410-3, Airman Competency/Proficiency Check, as applicable, and return it to the applicant.

NOTE:  At the time of publication, the Integrated Airman Certification and Rating Application (IACRA) is not able to process applications for ATP certificate with restricted privileges. Future updates to IACRA may allow for the processing of an airline transport pilot (ATP) certificate with restricted privileges. Until such upgrade, Form 8710-1 must be used for application file.

2)    Close PTRS.

B.    First Segment of a Two-Segment Flight Test.

1)     Mark, date, and sign the appropriate spaces on the FAA Form 8710-1, FAA Form 8410-1, or FAA Form 8710-3, as applicable, and return it to the applicant.

Note:  At the time of publication, IACRA is not able to process applications for ATP certificate with restricted privileges. Future updates to IACRA may allow for the processing of an ATP certificate with restricted privileges. Until such upgrade, Form 8710-1 must be used for application file.

2)    Clearly indicate on the job aid (use the appropriate job aid in Volume 5, Chapter 4, Section 4) those events that were not evaluated during the first segment by marking an “NE” (not evaluated) in the space provided. Sign, date and return the job aid to the applicant. The applicant should be instructed to give the application and job aid to the inspector or examiner conducting the airplane segment of the test. See paragraph 5-965 for guidance on documentation requirements for incomplete tests.

3)    If all increments of the practical test for a certificate or rating are not completed within two calendar months after the month the applicant began the test, the applicant must retake the entire practical test per Title 14 of the Code of Federal Regulations (14 CFR) part 61, § 61.39.
4)    Close PTRS.

5-963    SATISFACTORY PERFORMANCE. When the applicant has satisfactorily met all requirements for the certificate and/or rating sought, prepare FAA Form 8060-4, Temporary Airman Certificate, in accordance with the guidance in Volume 5, Chapter 1, Section 5.

A.    Enter the Correct Limitation(s). The limitations listed below may be issued simultaneously and/or in conjunction with operational restrictions.

1)    Enter “Holder does not meet the Pilot in Command aeronautical experience requirements of ICAO” if the applicant applied for a restricted privileges ATP certificate in accordance with § 61.160(a) , (b), (c), or (d). (Reference § 61.160(h).)
2)     Enter “Restricted in accordance with CFR § 61.167” if the applicant has not satisfied the age requirement of § 61.153(a)(1) and the aeronautical experience requirements of § 61.159. (Reference § 61.160(g).)

B.    Limitations Removal. An inspector will remove the restrictions placed on the ATP certificate when the applicant represents satisfactory evidence that he or she has met the regulatory requirements prescribed in part 61, as applicable, and FAA Form 8710-1 is completed. The applicant must complete section III – Record of Pilot Time on FAA Form 8710-1 to be issued an unrestricted ATP certificate.

C.    Issuing in a Foreign Country. If the pilot certificate is issued in a foreign country, see § 61.13(a) and part 187, appendix A.

D.    Verification. Verify that all information on the certificate is correct. Sign the Temporary Airman Certificate and direct the airman to sign line VII of the form.

E.    Document Retention Retain the original for the certification file and give the applicant the copy of the temporary certificate.

5-964    UNSATISFACTORY PERFORMANCE. If the applicant did not meet the applicable standards for the certificate sought, inform the applicant of the reasons for the unsatisfactory performance.

A.    Preparation. Prepare FAA Form 8060-5, Notice of Disapproval of Application, in accordance with the guidance in Volume 5, Chapter 1, Section 6.

B.    Indication of Unsatisfactory Performance. Indicate on the FAA Form 8060-5 the areas of operation and tasks that were unsatisfactorily performed on the practical test, and indicate those operations not performed during the practical test.

1)    All required areas of operation and tasks that the applicant was evaluated to be unsatisfactory must be listed on the FAA Form 8060-5.
2)    List the number of practical test failures by the applicant for this certificate or rating, in any available space on FAA Form 8060-5.
3)          An applicant for retesting may receive credit for those areas of operation and tasks completed satisfactorily on the previous practical examination(s). However, an inspector must reexamine the applicant on all areas of operation required for a pilot certificate or rating, if two calendar months have lapsed from the date of the initial practical test for the certificate or rating, as appropriate. An inspector may reexamine the applicant on any areas of operation required for a pilot certificate or rating, as appropriate, if the applicant demonstrates unsatisfactory proficiency or competence on a task that was previously evaluated satisfactory on a previous practical test.
4)    The inspector must date and sign the FAA Form 8060-5 and enter the district office routing code.

C.      Submission. Sign, date, and check the appropriate boxes on the form. Give the applicant a copy of the notice of disapproval and retain the original for the certification file.

D.      Document Retention. Retain the FAA Form 8710-1 and FAA Form 8410-1 or FAA Form 8410-3, as applicable and return all other submitted documents to the applicant.

5-965    INCOMPLETE TESTS. (Reference § 61.43.) If a practical test is discontinued, the applicant is entitled to credit for those areas of operation that were passed, but only if the applicant:

·    Passes the remainder of the practical test within the 60-day period after the date of the practical test was discontinued; and

·    Presents to the examiner for the retest the original letter of discontinuance.

SUCCESSFUL ORAL OR FIRST SEGMENT OF TWO-SEGMENT FLIGHT TEST

[   ] Mark, date, and sign FAA Form 8410-1, Airman Proficiency/Qualification Check, FAA Form 8410-3, Airman Competency/Proficiency Check, or FAA Form 8710-1, Airman Certificate and/or Rating Application as applicable. Return to applicant.

[   ] Complete FAA Form 8000-36, PTRS Data Sheet, and forward to the Flight Standards District Office (FSDO).

UNSUCCESSFUL APPLICANTS

[   ] Complete FAA Form 8060-5, Notice of Disapproval of Application, in duplicate:

·     The events failed must be indicated.

·     If the check is not to be entirely repeated, those

 events not yet accomplished must be annotated.

·     Enter oral and simulator date, as applicable.

·     Indicate 1st or 2nd failure.

[   ] Give the duplicate of the FAA Form 8060-5 to the applicant.

[   ] Mark, date, and sign FAA Form 8410-1, FAA Form 8410-3, or FAA Form 8710-1 as applicable.

[   ] Cross out back of application form with diagonal line and annotate line with the words “Job aid used.”

[   ] Complete FAA Form 8000-36, PTRS Data Sheet.

[   ] Attach the original Notice of Disapproval and the PTRS Data Sheet to the application and forward the paperwork to the FSDO.

SUCCESSFUL COMPLETION OF FLIGHT TEST

[   ] Mark, date, and sign FAA Form 8410-1, FAA Form 8410-3, or FAA Form 8710-1 as applicable.

[   ] Cross out back of application form with diagonal line and annotate line with the words “Job aid used.”

[   ] Prepare an FAA Form 8060-4, Temporary Airman Certificate, in duplicate. Enter ICAO restrictions, if required.

[   ] Give applicant the duplicate copy of the FAA Form 8060-4.

[   ] Complete PTRS Data Sheet.

[   ] Attach the following to the application and forward the paperwork to the FSDO:

[   ] Airman Written Test Report (AC Form 8080-2).

[   ] Notice of Disapproval (Form 8060-2) if applicable.

[   ] Superseded airman’s certificate.

[   ] PTRS Data Sheet (FAA Form 8000-36).
