Purpose:

To generate a Safety Recommendation to correct an identified safety deficiency in the National Airspace System (NAS) in accordance with FAA Order 8020.11, Aircraft Accident and Incident Notification, Investigation, and Reporting.

1.          Safety Deficiency identified within the National Airspace System

2.          Prepare a memorandum that includes the following;

2.1      Description of the safety deficiency

2.2      Sufficient detail and/or substantiating information

2.3     Recommended safety recommendation, one for each deficiency identified

2.4      Specify how the recommendation mitigates the safety deficiency

3.          Memorandum review by supervisor or subject matter expert

4.          Send memorandum to Federal Aviation Administration, Office of Accident Investigation, Recommendation and Analysis Division, AVP-400
